A transmissive pulse sensor: an infrared LED on one side, a phototransistor facing it on the other, and your fingertip in the gap. Each heartbeat pushes a little more blood through the finger, which blocks slightly more light, and the phototransistor's analog output wobbles in time with it. Read it on an analog pin, then smooth and peak-detect in software to get beats per minute. A cheap, hands-on way to learn photoplethysmography - it is not a medical instrument.
