Two parts joined by a short lead: a forked probe that goes into the soil, and a small LM393 board that reads it. Current passes between the prongs, wet soil conducts better than dry, and you get an analog A0 that changes with moisture plus a digital D0 that trips at the point you dial in on the trim pot. One caution worth planning around - resistive probes corrode when left powered in damp soil, so drive the probe from a GPIO pin and switch it on only while you take a reading.
